＜p＞It is indeed ironical that in the absence of a complete knowledge of Pathophysiology, clinical cardiologists are left with no choice but to do the best they can to help the patient with the armamentarium of drugs at their disposal. But nothing could be further from truth than to treat the diagnosed end point of a disease process without a full understanding of its patho physiology. This point was eloquently made by Dr. Arnold Katz in his Presidential Address (Chapter 1) at the 8th Annual Meeting of the American Section of the International Society for Heart Research held in Winnipeg, Canada, July 8-11, 1986. This volume represents a part of the scientific proceedings of this Meeting. From a reading of this treatise it will become evident that discoveries of newer scientific facts as well as a better understanding of pathophysiology are continuously influencing/ improving our therapeutic approaches in modern medicine. In this book, latest biochemical, physiological and pharmacological findings on different experimental models such as Myocaridal hypertrophy, Hypertension and heart failure, Diabetes, Cardio myopathies and Cardiac function in shock are described by internationally recognised experts. Hopefully information presented here will provide another building block to the edifice of Science of Cardiology which we all are trying to create. ＜p＞Safety and efficacy of radiopharmaceuticals are elements of great importance in nuclear medicine. Since the first meeting in 1965 in Oak Ridge with the title Radiopharmaceuticals tremendous developments have taken place. In 1965 the whole technetium-99m area was just in its very beginning. Safety and efficacy of the non-radioactive pharmaceuticals have attracted great attention during the last 10 years and so have similar aspects of radiopharmaceuticals during the later years. Regulatory agencies are extending their work also to the preparation of radiopharmaceuticals at hospitals and to requirements for registration of radiopharmaceuticals. In a fast developing field there might be tendencies to confrontation between interests and there have certainly been some tendencies to put undue restrictions on the use of radio pharmaceuticals due to the lack of understanding between the industry and the regulatory authorities and between regulatory authorities and hospitals. Much of this may have been due to lack of information and certainly is due to the lack of fundamental scientific knowledge in many radiopharmaceutical aspects. A fast and safe introduction of new radio pharmaceuticals and the proper handling of these requires a lot of development work, but also an understanding of how general principles from the non-radioactive drug field may be sensibly transformed into the radiopharmaceutical area. ＜p＞The ability to measure accurately the hormones regulating calcium homeosta sis is the fundamental first step toward understanding the roles these hormones play in health and disease. Techniques for such measurements have only been available for the past 10 years or so and remain in a state of rapid development. Sensitive parathyroid hormone (PTH) radioimmunoassays appeared in the early 1970s, and with them came a whole new appreciation for the prevalence and implications of hyperparathyroidism, primary or secondary, in the popu lation. The calcitonin (CT) radioimmunoassay came later and achieved rapid success in the. diagnosis of a previously poorly understood cancer, medullary carcinoma of the thyroid, frequently associated with the familial multiple endo crine neoplasia type 2 syndromes (a and b). As the sensitivity of the calcitonin radioimmunoassay has improved, our understanding of the role of calcitonin in normal physiological processes has increased. The knowledge that vitamin D must be metabolized to produce its biologic effects is only 15 years old. This has had profound implications in our understanding of a variety of metabolic bone, kidney, and gastrointestinal diseases. Assays to measure the major cir culating form of vitamin D, 25-hydroxyvitamin D, were described 10 years ago. Assays for the other metabolites, in particular, 1,25-dihydroxyvitamin D, were described even more recently.
